FREQUENTLY ASKED QUESTIONS FOR PHYSICAL THERAPY

•What do I bring to my evaluation?

Bring your identification and insurance card, the online paperwork if you filled it out prior to attending, and your script(if your physician gave you a hard copy). If you plan to fill out your paperwork the day of your appointment, plan to arrive 30 minutes ahead of your treatment time to give you ample amount of of time to check-in and complete everything. You may/may not have a co-pay for the first visit so please provide a form of monetary transaction just in case.

•What do I wear?

Please wear loosely-fitted clothing, workout gym attire if you have, and make sure your injured area can be exposed. At times, the therapist may have to address surrounding areas above and below the injured site. There is a bathroom and private room available for you to change if necessary. Please note the gym area’s temperature ranges from 67 degrees to 70 degrees, and you may need to layer clothing.

•How long is the evaluation?

Evaluations are dependent per patient and can range from 30 minutes to 60 minutes. Please alert your therapist if you have any time restraints.

•Why is there a cancellation fee if I don’t attend my appointment or I did not notify the office more than 24 hours in advanced?

We promote accountability with our patients’ scheduling and the time slots are highly valued for people who want to get better. We understand things happen, but hope that you also understand that other people would like to get in for therapy as soon as possible.
